DACC Transcript Evaluation Request Form

Transcript Evaluation Request may take up to 4 weeks to process. Students will be notified by DACC student email when an evaluation request is completed.
The DACC Transcript Evaluation Request Form is to be completed only after official transcript from any other institution(s) of higher education that you previously attended have already been sent to the records department. For information on how to send transcripts to DACC please visit https://dacc.edu/ar/transcript-send.

If you switch your program of study it is recommended to complete a new Transcript Evaluation Request form as classes may apply differently pending the program. If you have any questions regarding transcript evaluations please email records@dacc.edu.
