Read all instructions carefully. Any incomplete or late applications will be rejected. The following is required to complete a successful application.
1. Complete the Danville Area Community College application online and new student orientation for admission if not currently enrolled already in college credit courses at DACC. (DACC ID required to complete application)
2. $35.00 Application fee made payable to DACC at the cashier’s office. The Health Professions Office cannot accept payment of any type. Once payment is made at the cashier’s office, a receipt of the payment is to be submitted with this application.
3. Proof of Residency: Submit a copy of valid driver's license, state identification card, or a voters registration card with your current address.

5. Official transcripts from all colleges or universities previously attended must be on file with DACC Records Office. If currently taking courses, please wait until completed to apply.
6. Submit unofficial or copies of all transcripts, including DACC transcripts. You are responsible for obtaining copies of transcripts and submitting with this application.
7. If applicable, provide proof of active unencumbered health care related licenses/certificates.
8. Completed ATI-TEAS ® Test – (Performance Profile from ATI student account must be submitted to the Nursing Program with the application). Must score proficient or higher overall and 50% on English to apply. Results must be included with application. Scores on each section must be visable on the form. ATI-TEAS scores are valid for 2 years.
9. If accepted, I understand I must attend scheduled in-person orientations on June 23rd and August 11th, 2025.
10. Students considering a program of study that leads to professional licensure, endorsement or certification in Nursing should contact the appropriate licensing agency in the state where you plan to be licensed prior to enrollment at Danville Area Community College to ensure the coursework meets that state's requirements. Admission to the Nursing Program is competitive and the number of students accepted is limited. Meeting the minimum requirements DOES NOT guarantee acceptance into the program.
